This form should be used to make an application for the disclosure by the Tribunal of documents held by it pursuant to its statutory function under the Solicitors Act 1974 to hear and determine applications made to it under that Act.

Applications to the Tribunal must be made using this form, supported by a statement setting out the allegations which the respondent to the application is required to answer and the facts and matters in support of the application and each allegation contained in it.
